fix(holdingk-db): order_machine_config 补 id PK 与审计人字段
对齐 cloth_ticket_runtime：自增 id PK + uk_order_machine_id；补 create_by/update_by；V55 幂等 ALTER。验证：V53→V54→V55；已跑旧 V53 环境跑 V55（checksum 失败则 repair）。
